San Felipe, Zambales – In line with the Department of Education’s commitment to ensure the accuracy and completeness of learner records, San Felipe District conducted the Checking of School Forms on April 4 and 7, 2025, held at San Felipe Elementary School West.

The activity was conducted in compliance with DepEd Order No. 11, s. 2018, titled “Guidelines on the Preparation and Checking of School Forms”. It was spearheaded by Mrs. Whea De Jesus Dayap, Coordinating Principal of the San Felipe District, under the supervision of Mrs. Genna S. Manglicmot, Planning Officer III.

During the two-day checking, priority grade levels including Kindergarten, Grades 1, 6, 7, 10, 11, and 12 were scheduled on April 4, 2025, considering the total school size based on the combined enrollment in these key transition levels. All other grade levels followed on April 7, 2025.

One of the key findings during the checking was the use of barangay certification in place of birth certificates for some learners. This highlighted the need for schools to assist affected families in acquiring the necessary documents for late registration. Ensuring that learners have official birth records is vital for establishing their legal identity and for future academic and legal transactions.

More than just a compliance activity, the checking of school forms served as an important opportunity to assess the consistency, accuracy, and completeness of learners’ data and supporting documents. The district aims to continue strengthening data management practices and providing support mechanisms for learners who lack required documentation.

Through this activity, the San Felipe District reaffirms its dedication to upholding the integrity of school records and promoting inclusive education for all learners.
